PLEASE NOTE: - The Medicine Shoppe of Butler has provided Immunization Consent forms that will be available at any All Saints Parish (Butler) churches, or by clicking here.
- You must fill out a consent form, make a copy of your insurance card, and return the information to The Medicine Shoppe Pharmacy directly (not All Saints Parish) by October 9, 2026.
- Consent forms can be dropped off at The Medicine Shoppe Pharmacy in person, mailed or emailed.
